Start your Self-Referral

Starting your self-referral is easy.  

Simply phone 403-986-2446 and be sure to have your Alberta Health Care number ready. We will ask you a few questions to make sure this clinic is the best fit for your injury.

We will then book you in to see one of our physiotherapists within a few days for a complete assessment.

There is a fee of $100 for your initial triage assessment with physiotherapy which may be covered under your health benefits.  The diagnosis and management of your injury will then be reviewed with an MSK Physician.  Following the assessment if it is recommended that you see an Orthopedic Surgeon, this will be arranged at no additional cost.

If you have seen your family doctor and he/she feels that you need to be seen by an Orthopedic Surgeon or MSK Physician, then they are encouraged to make the referral on your behalf.  You will then be triaged accordingly.  

Please click here for a form your Family Doctor can fill out and fax to us.  Once we receive this form, we will contact you directly and arrange for a physiotherapy assessment within a few days.
